Detailed Request for Mentorship and Training Support

As a Registered Midwife working at Munini District Hospital in Rwanda, I encounter a wide range of complex obstetric and gynecological conditions that often require advanced skills, collaborative decision-making, and surgical interventions. Below is a detailed outline of the areas where I seek mentorship, partnership, and possible in-person or virtual training through InterSurgeon.

Obstetric Surgical & Emergency Procedures

Obstructed Labour: I aim to build expertise in identifying early signs of obstructed labour, timely referral, and hands-on surgical management to reduce maternal-fetal complications in resource-limited rural settings.

Abnormal Placentation: I seek advanced clinical support in diagnosing and managing placenta previa, placenta accreta/increta/percreta, and postpartum hemorrhage associated with abnormal placental attachment.

Management of Multiple Pregnancy Deliveries: I wish to refine my skills in conducting safe deliveries for twins or more, including preparation, timing of delivery, and postnatal care.

Breech Delivery (Vaginal & Caesarean): Training in decision-making for trial of vaginal breech delivery versus caesarean section, and hands-on mentorship during these deliveries, would improve maternal safety and outcomes.

Caesarean Hysterectomy: I seek mentorship in recognizing indications and performing or assisting in life-saving caesarean hysterectomies, particularly in emergency situations like uterine rupture or uncontrolled bleeding.

Wound Debridement: Additional skills in managing surgical site infections, wound healing, and proper debridement techniques would enhance post-operative care in our facility.

Episiotomy and Tear Repair: I aim to master repair techniques for complex episiotomies, vaginal tears, and third/fourth-degree perineal tears including anal sphincter involvement.

Operative Deliveries

Spontaneous Vertex Delivery: Although I am experienced in SVDs, I would appreciate feedback and training in identifying subtle signs of complications during labor progression.

Simple Caesarean Section: I am interested in mentorship to strengthen my theoretical and hands-on understanding of all stages of a caesarean section, including managing complications.

Operative Vaginal Delivery (Forceps/Ventouse): These are rarely practiced in our rural setting. I seek practical demonstrations, simulations, and mentorship to safely implement these techniques where appropriate.

Vulval Disease

Dermatoses (Lichen sclerosus, Lichen planus, etc.): I request clinical guidance in diagnosis, differential diagnosis, and long-term management of chronic vulval skin conditions.

Paget’s Disease of the Vulva & VIN: As these are rare and often misdiagnosed, I hope to gain knowledge on presentation, biopsy interpretation, and management strategies including conservative and surgical approaches.

Colposcopy and Cervical Pathology

HPV and CIN: I am keen to learn best practices for colposcopy, HPV screening, and managing cervical intraepithelial neoplasia (CIN), especially in low-resource contexts.

Minimally Invasive Cervical Cancer: I would benefit from exposure to techniques and protocols for early-stage cervical cancer, especially how it is managed surgically or conservatively in settings with limited access to oncology centers.

I hope to develop practical surgical and clinical competencies through mentorship from experienced professionals via InterSurgeon. Whether through in-person training, remote case discussions, simulations, or shared educational materials, I am fully committed to using this knowledge to improve patient outcomes, train other midwi
